Zusammenfassung
Die Abrechnung von Laborleistungen ist für das wirtschaftliche Überleben der Labore ganz wesentlich. Zunehmend komplexe organisatorische Strukturen müssen auch in der Abrechnung berücksichtigt werden. Laborinformationssysteme (LIS) sind in allen medizinischen Laboren vorhanden und müssen auch die Aufgaben der Abrechnung unterstützen. In dem vorliegenden Artikel beschreiben wir den aktuellen Stand der Abrechnung von Laborleistungen, Besonderheiten der Labormedizin und relevante Unterschiede zwischen der Abrechnung im Laborsystem selbst bzw. der Abrechnung in einem Drittsystem. Zwar scheint die Abrechnung im Patientenverwaltungssystem meist erst der einfachere Weg, so ist es aber doch so, dass auch hier viele Punkte zu beachten sind. Insbesondere die Laboreinrichtungen, die keine externen Einsendungen bekommen, können hier besonders leicht zum Ziel kommen. Je vielfältiger aber die Einsenderstruktur eines Laboratoriums ist, desto eher ist die Durchführung der Abrechnung im Laborsystem selbst zu empfehlen. Anhand einer Checkliste kann bei der Auswahl eines Systems geprüft werden, ob es den jeweiligen Anforderungen entspricht. Die Abrechnung von Laborleistungen mit den Kassenärztlichen Vereinigungen ist momentan besonders von Veränderungen betroffen. Es bleibt abzuwarten, welche Änderungen die Neugestaltung der Gebührenordnung für Ärzte bringen wird.
Abstract
Accounting services of a medical laboratory are important for an organisation to be profitable. Increasingly complex structures of an organisation need to be considered. Many medical laboratories use laboratory information systems (LISs) that have to support accounting. Here, we describe the current state of accounting in medical laboratories. We focus on special situations in laboratory medicine and on relevant differences between accounting with LISs and accounting with third party or patient administration systems. Accounting with a patient administration system seems to be the simplest method, but special circumstances need to be considered. The more complex the requesting structure is, the more accounting with LISs is recommended. A checklist will provide the reader with the opportunity to choose a matching method of accounting for each individual situation of the laboratory in question. In particular, the “Kassenärztliche Vereinigungen”, responsible for accounting with compulsory health insurances, is currently undergoing large-scale changes. In the near future more changes are to be expected, particularly with the new medical fee regulation.
