Abstract
Zusammenfassung Infektionen sind die Haupt- Mortalitätsursache bei ZNS- verletzten Patienten nach Schlaganfall, traumatischer Hirnverletzung sowie nach Rückenmarkverletzung. Sie beeinträchtigen die Wundheilung sowie die neurologische Restfunktion. Eine lange Suche nach den zu Grunde liegenden Mechanismen hat zur Identifikation einer ZNS-gesteuerten, sekundären Immunsuppression geführt (CIDS). Diese betrifft sowohl die unspezifische als auch die spezifische Immunabwehr. ZNS-Verletzungen induzieren einen autoregulativen, negativen Feedback-Mechanismus, der zu einer Freisetzung von anti-entzündlichen Zytokinen/Faktoren wie IL-10, TGF-β, Neuropeptiden (α-melanocyte stimulating hormone (MSH), β-endorphin) führt. Zudem wird hauptsächlich durch die freigesetzten Zytokine TNF-α und IL-1β eine Aktivierung des Hypothalamus-Hypophysen- Nebennieren-Systems (HHNS) bewirkt. Die HHNS-Aktivierung stimuliert das symphatisch-autonome Nervensystem und führt zu einer Katecholaminliberation. Die freigesetzten Katecholamine bewirken die Deaktivierung und den Zelltod von Monozyten und Lymphozyten in Verbindung mit einer Reduktion der dendritischen Zellzahl. Die post-traumatische neuroendokrine Sympathikus-Aktivierung stellt einen ersten wichtigen Schritt dar, um die neurophysiologischen Grundlagen der induzierten Immundefizienz zu verstehen und damit besonders die Akutbehandlung nach ZNSVerletzung zu optimieren. Die durch ZNS-Verletzung verursachte prä-ganglionäre Sympathikus-Schädigung (Teil des ZNS-eigenen autonomen Nervensystems) verifiziert das ZNS als immunprivilegiertes Organ - allerdings in neuem Kontext.

Abstract
Zusammenfassung Operative Eingriffe in den Basalganglien zur Therapie schwerer Bewegungsstörungen werden bereits seit über 50 Jahren durchgeführt. Seit der Ablösung der klassischen läsionellen Operationstechnik durch die chronische Tiefenhirnstimulation haben stereotaktische Operationen in den letzten Jahren als Behandlungsalternative wieder an Bedeutung gewonnen. Der Operationserfolg hängt dabei von einer präzisen Platzierung der zur Stimulation verwendeten Elektrode am Zielort ab. Für die millimetergenaue Orientierung in den Tiefenstrukturen des Gehirns zeichnet man Nervenzellsignale mit Hilfe von Mikroelektroden auf. Diese Technik ermöglicht die Darstellung der subkortikalen Kerngrenzen, die Abgrenzung funktioneller Kernregionen sowie die Bestimmung pathologischer neuronaler Aktivitätsmuster, und ist damit im klinischen Kontext von grosser Bedeutung. Intraoperative Mikroelektrodenableitungen ermöglichen aber auch einzigartige Einblicke in die neuronalen Vorgänge in Tiefenstrukturen des menschlichen Gehirns. Neben einem Überblick über den praktischen Einsatz von Mikroelektroden in den Basalganglien des Menschen soll skizziert werden, wie mit dieser Technik aktuelle, aus der tierexperimentellen Physiologie herrührende Hypothesen getestet werden können.

Abstract
Zusammenfassung Erbliche Neuropathien sind genetisch bedingte Erkrankungen des peripheren Nervensystems. In dieser Krankheitsgruppe führt ein zugrundeliegender genetischer Defekt häufig zur Fehlfunktion von myelinisierenden Schwannzellen. Neben der Dysund Demyelinisierung der Nerven ist die Degeneration der betroffenen Axone und der denervierten Muskelfasern der wichtigste Grund für die charakteristische Muskelschwäche bei dieser Krankheit. Die mit Abstand häufigste Neuropathie wird durch eine 1.5Mb große Duplikation auf Chromosom 17 hervorgerufen (CMT1A). Die Überexpression eines in dieser Region enthaltenen Gens für ein Strukturprotein der Myelinscheide ist für die Demyelinisierung und für sekundäre axonale Schäden verantwortlich. Dies lässt sich durch Tiermodelle mit transgener Überexpression des PMP22 Gens formal beweisen. Solche Tiermodelle sind nicht nur für die Analyse des Krankheitsmechanismus wichtig. Sie sind, wie jetzt gezeigt, auch für erste experimentelle Therapien der CMT1A unverzichtbar.
