Abstract

Background Alzheimer’s disease (AD) is a progressive neurodegenerative disease. AD is the main cause of dementia worldwide and aging is the main risk factor for developing the illness. AD classical diagnostic criteria rely on clinical data. However, the development of a biological definition of AD using biomarkers that reflect the underling neuropathology is needed. Content The aim of this review is to describe the main outcomes when measuring classical and novel biomarkers in biological fluids or neuroimaging. Summary Nowadays, there are three classical biomarkers for the diagnosis of AD: Aβ42, t-Tau and p-Tau. The diagnostic use of cerebrospinal fluid biomarkers is limited due to invasive collection by lumbar puncture with potential side effects. Plasma/serum measurements are the gold standard in clinics, because they are minimally invasive and, in consequence, easily collected and processed. The two main proteins implicated in the pathological process, Aβ and Tau, can be visualized using neuroimaging techniques, such as positron emission tomography. Outlook As it is currently accepted that AD starts decades before clinical symptoms could be diagnosed, the opportunity to detect biological alterations prior to clinical symptoms would allow early diagnosis or even perhaps change treatment possibilities.

Abstract

Resumen Objetivos La enfermedad de Alzheimer (EA) es una enfermedad neurodegenerativa. La EA es la principal causa de demencia en el mundo, siendo el envejecimiento el principal factor de riesgo. Los criterios diagnósticos para la enfermedad de Alzheimer suelen basarse en datos clínicos. No obstante, es necesario establecer una definición biológica de la enfermedad de Alzheimer basada en biomarcadores que reflejen la neuropatología subyacente. Contenido El objetivo de esta revisión es presentar los resultados obtenidos en la medición de biomarcadores nuevos y ya conocidos en los fluidos biológicos o en neuroimágenes. Resumen Actualmente se emplean tres biomarcadores para el diagnóstico de la enfermedad de Alzheimer_Aβ42, t-Tau y p-Tau. El uso diagnóstico de biomarcadores en el líquido cefalorraquídeo (LCR) presenta algunas limitaciones debido a que la obtención invasiva mediante punción lumbar puede provocar efectos secundarios. La práctica más común en los centros clínicos es la medición en plasma o suero, ya que es mínimamente invasiva y, en consecuencia, se puede obtener y procesar con mayor facilidad. Las dos principales proteínas implicadas en el proceso patológico, Aβ y Tau, se pueden visualizar empleando técnicas de neuroimagen como la PET. Perspectivas Dado que está ampliamente aceptado que la enfermedad de Alzheimer comienza décadas antes de que se diagnostiquen los primeros síntomas clínicos, la detección de alteraciones biológicas previa a la aparición de la sintomatología clínica permitiría su diagnóstico precoz o incluso abriría la puerta a nuevas opciones terapéuticas.

Abstract

The objective of this paper is to share some considerations about the management of postanalytical processes in relation to the review, reporting and release of test results in accordance with UNE-EN ISO 15189:2013 Standard requirements. The scope of this paper includes postanalytical activities and the personnel involved (laboratory management and staff). We describe the criteria and information required to review and validate analytical results and ensure that clear reports are sent to requesters. These criteria also guarantee that results are transcribed in a reliable way and that all necessary information is provided for the correct interpretation of results. Likewise, the requirements for the correct release of laboratory results are described, with special emphasis on the release of alarming or critical results. In some European countries, clinical laboratories are required to hold partial or full ISO 15189 accreditation, which is a global trend. Therefore, understanding ISO 15189 requirements is imperative for a progressive and more effective implementation of the Standard.

Abstract

Resumen El objeto de este trabajo es establecer unas consideraciones para facilitar la gestión del proceso posanalítico respecto a la revisión, notificación y comunicación de los resultados, de acuerdo con los requisitos de la Norma UNE-EN ISO 15189:2013. El ámbito de aplicación incluye las actividades del proceso posanalítico del laboratorio clínico, así como el personal implicado en él (dirección y personal del laboratorio). Se indican los criterios y la información necesaria para realizar la revisión y validación de los resultados de las pruebas analíticas y así enviar a los destinatarios informes claros, asegurando siempre una transcripción fidedigna de los resultados e incluyendo toda la información necesaria para su correcta interpretación. Asimismo, se describen los requisitos para una correcta comunicación de los resultados del laboratorio, haciendo especial hincapié en la comunicación de aquellos resultados alarmantes o críticos. En algunos países de Europa es obligatoria la acreditación, total o parcial, de los laboratorios clínicos, siguiendo la Norma ISO 15189 y esta parece ser la hoja de ruta marcada en otros muchos países. Por ello, es indispensable la comprensión de sus requisitos para realizar una implementación progresiva y más fácil.

Abstract

Objectives Schistosomiasis is a blood fluke parasitic illness affecting human lives in rural endemic areas. This study evaluated the performance of Schistosoma ICT Ig G - IgM for screening urinary schistosomiasis in Nigeria. Methods Three hundred and seventy four (374) urine samples were examined. Reagent strips, urine filtration and Schistosoma ICT Ig G - IgM were used for analysis. Schistosoma ICT Ig G - IgM used 2 mL of each serum for serological examination. Then, 3 mL of each preserved serum was sent to LDBIO Diagnostics, France for re-examination with Schistosoma ICT IgG-IgM and confirmation with SCHISTO Western blot (WB) IgG. The performance of the index tests was determined using sensitivity (Se), specificity (Sp), positive predictive value (PPV), negative predictive value (NPV) and positive likelihood ratio (PLR). The Youden index (YI) and diagnostic accuracy (DA) were used to determine the accuracy of each test. The statistical significance was at p-value ≤0.05. Results The test had a sensitivity of 94.9%, specificity of 63.9%, positive predictive value of 72.4%, negative predictive value of 92.6%, and positive likelihood ratio of 2.62. Schistosoma ICT Ig G - IgM had a good Cohen’s kappa index (κ=0.68), good Youden index (YI=0.58) and good diagnostic accuracy (DA=0.78). Conclusions Schistosoma ICT Ig G - IgM has proven to be the best technique for the screening of urinary schistosomiasis in Nigeria.

Abstract

Resumen Objetivos La esquistosomiasis es una enfermedad parasitaria causada por gusanos del género esquistosomazonas que afecta a humanos en zonas rurales donde el parásito es endémico. En este estudio se evalúa el rendimiento diagnóstico del test rápido Schistosoma ICT IgG-IgM para la detección de la esquitosomiasis urinaria en Nigeria. Métodos Se analizaron un total de 374 muestras de orina. Para el análisis de las muestras se emplearon tiras reactivas, filtración de orina y el test de Schistosoma ICT IgG-IgM . Se emplearon 2 mL de suero para el análisis serológico con el test Schistosoma ICT IgG-IgM . A continuación, se enviaron 3 mL de cada muestra sérica a LDBIO Diagnostics (Francia) para un segundo análisis con el test Schistosoma ICT IgG-IgM y confirmación mediante IgG SCHISTO Western blot (WB). El rendimiento de la prueba evaluada se determinó calculando la sensibilidad (Se), especifidad (Sp), valor predictivo positivo (VPP), valor predictivo negativo (VPN) y la razón de verosimilitud positiva (LR+). La exactitud de cada prueba se determinó calculando el Índice de Youden (IY) y la exactitud diagnóstica (ED). El nivel de significación estadística se determinó en un valor p≤0,05. El test Schistosoma ICT IgG-IgM detectó la infección en el 63,9% de las muestras. Resultados El test mostró una sensibilidad del 94,9%, una especifidad del 63,9%, un valor predictivo positivo del 72,4%, un valor predictivo negativo del 92,6% y una razón de verosimilitud de 2,62. La técnica Schistosoma ICT IgG-IgM mostró un buen índice kappa de Cohen (κ=0,68), un buen Índice de Youden (IY=0,58), así como una buena exactitud diagnóstica (ED=0,78). Conclusiones Schistosoma ICT IgG-IgM ha demostrado ser la mejor técnica para la detección de la esquistosomiasis urinaria en Nigeria.

Abstract

Objectives This work aimed to assess the diagnostic validity of two approaches for the screening of gestational diabetes mellitus (GDM) with less discomfort for pregnant women. Methods A prospective diagnostic validation study was conducted with 2007 pregnant women. According to risk factors for GDM, women were classified into high-risk and low-risk groups. The current diagnostic procedure, based on oral glucose overload, was followed; simultaneously HbA 1c was tested and an algorithm combining both biomarkers was applied. Results In the low-risk group, the Glucose challenge test (GCT) showed a higher area under the curve (AUC 0.953; 95% CI 0.915–0.992) than the HbA 1c test (0.688; 95% CI 0.541–0.834). The best GCT cut-off, 153.5 mg/dL (8.52 mmol/L), showed higher diagnostic validity than that for HbA 1c , 28 mmol/mol (4.75%), and that the algorithm using both tests. In the high-risk group, the GCT showed better diagnostic performance than the HbA 1c and the algorithm; the optimal GCT cut-offs were higher than those recommended in current protocols. 13th week: GCT AUC 0.882 (95% CI 0.843–0.921), HbA 1c AUC 0.624 (95% CI 0.562–0.686), GCT cut-off 140.5 mg/dL (7.8 mmol/L), HbA 1c cut-off 33 mmol/mol (5.15%). 24th week: GCT AUC 0.944 (95% CI 0.925–0.962), HbA 1c AUC 0.642 (95% CI 0.575–0.709), GCT cut-off, 145.5 mg/dL (8.08 mmol/L), HbA 1c cut-off 29 mmol/mol (4.85%). Conclusions The GDM diagnostic approach using as the first step the GCT with higher cut-offs showed the best diagnostic validity. Applying these thresholds, 55.6 and 13.7% of 100 g. Oral glucose overloads would have been avoided in low-risk and high-risk pregnant women.

Abstract

Resumen Objetivos El objetivo de este trabajo es evaluar la validez diagnóstica de dos métodos de cribado de diabetes mellitus gestacional (DMG). Métodos Se realizó un estudio prospectivo de validación diagnóstica en 2007 embarazadas. Las participantes fueron asignadas al grupo de alto o bajo riesgo, dependiendo de los factores de riesgo que presentaran. Se realizó una prueba de HbA 1c simultáneamente al protocolo estándar basado en el test de O’Sullivan (TOS). Posteriormente, se aplicó un algoritmo que combinaba los resultados de ambos biomarcadores. Resultados En el grupo de bajo riesgo, el TOS mostró un área bajo la curva mayor (AUC 0,953; IC95% 0,915–0,992) que la prueba de HbA 1c (0,688; IC 95% 0,541–0,834). El mejor punto de corte del TOS, 153,5 mg/dL (8,52 mmol/L), mostró mayor validez diagnóstica que el de la HbA 1c , 28 mmol/mol (4,75%) y que el algoritmo basado en ambas pruebas. En el grupo de alto riesgo, el TOS mostró mejor rendimiento diagnóstico que la HbA 1c y el algoritmo. Los puntos de corte óptimos para el TOS fueron mayores que los recomendados en los protocolos actuales. Semana 13: TOS AUC 0,882 (IC 95% 0,843–0,921), HbA 1c AUC 0,624 (IC 95% 0,562–0,686), punto de corte para el TOS 140,5 mg/dL (7,8 mmol/L), punto de corte para la HbA 1c 33 mmol/mol (5,15%). Semana 24: TOS AUC 0,944 (IC 95% 0,925–0,962), HbA 1c AUC 0,642 (IC 95% 0,575–0,709), punto de corte para el TOS, 145,5 mg/dL (8,08 mmol/L), punto de corte para la HbA 1c 29 mmol/mol (4,85%). Conclusiones El método para el cribado de DMG basado en el test de O’Sullivan con valores límite superiores a los recomendados fue el que mostró mejor validez diagnóstica. Si se hubieran aplicado estos umbrales, se habría evitado la prueba de sobrecarga oral de glucosa con 100 g al 55,6% y al 13,7% de las gestantes de bajo y alto riesgo.

Abstract

Objectives To assess the effectiveness of incorporating hygienic-dietary recommendations in laboratory reports in reducing the incidence of renal colic (RC). A study was performed to compare the incidence of RC in two groups of patients who had suffered at least a crystalluria event associated with the risk of urolithiasis. Recommendations were only incorporated in the laboratory reports of one group. Methods A retrospective observational study. The study sample was composed of patients who had at least an episode of crystalluria associated with a higher risk of urolithiasis. The laboratory reports of patients in Group A (n=1,115), treated in 2017, did not include any hygienic-dietary recommendations, whereas patients in Group B (n=1,692), treated in 2018, received hygienic-dietary recommendations through their laboratory reports. χ 2 and Mann-Whitney U test were used to assess differences based on sex, age, and type of urinary crystals. Results The incidence of RC was 2.02 times higher in group A (2.24%) than in group B (1.12%). No significant differences were observed in the incidence of RC based on the type of urinary crystal. The incidence of RC was substantially higher in patients who suffered at least an event of crystalluria associated with a higher risk for urolithiasis as compared to the general population during the same period (0.46%, consistently with the incidence rates reported in the literature). Conclusions The incorporation of messages alerting on the risk of urolithiasis and the inclusion of hygienic-dietary recommendations in laboratory reports may be useful for reducing the incidence of RC.

Resumen Objetivos El presente estudio evalúa la efectividad de incorporar medidas higiénico-dietéticas en los informes de laboratorio comparando la incidencia de cólicos nefríticos (CN) en dos grupos de pacientes que presentaron, al menos, un evento de cristaluria asociada con riesgo litogénico, indicando solo a uno de ellos la introducción de estas recomendaciones. Métodos Estudio observacional retrospectivo. Grupo A: 1.115 pacientes que en el año 2017 presentaron al menos un episodio de cristaluria asociada con riesgo litogénico y no recibieron recomendaciones higiénico-dietéticas con el informe de laboratorio. Grupo B: 1.692 pacientes que en el año 2018 presentaron al menos un episodio de cristaluria asociada con riesgo litogénico y recibieron recomendaciones higiénico-dietéticas con el informe de laboratorio. Las diferencias entre grupos según sexo, edad y tipo de cristales urinarios fueron analizadas mediante pruebas χ 2 y U de Mann–Whitney. Resultados La incidencia de CN fue de 2,24% en el grupo A v 1,12% en el grupo B. Los pacientes del grupo A presentaron 2,02 veces más CN que los del grupo B. No se encontraron diferencias significativas en la incidencia de CN según el tipo de cristal urinario. La incidencia de CN en pacientes que presentaron al menos un evento de cristaluria asociada con riesgo litogénico durante el año de estudio fue mucho mayor que la de la población general en el mismo período (0,46%, similar a las publicadas previamente). Conclusiones La introducción de mensajes que alertan sobre el riesgo litogénico junto con la inclusión de medidas higiénico-dietéticas en los informes de laboratorio son herramientas para tratar de reducir la incidencia de CN.

Abstract

Objectives Immunoassays used to assess thyroid function are vulnerable to different types of interference that may affect clinical decision-making. Case presentation We report the case of a 37-year-old woman who developed iatrogenic hypothyroidism after having received radioiode therapy who visited our hospital for her annual checkup. The patient was asymptomatic, without signs suggestive of thyroid disease. However, laboratory analysis proved otherwise: thyrotropin (TSH) 7.75 mU/L, thyroxine (FT4) >7.7 ng/dL. Conclusions The inconsistency between her clinical symptoms and the biochemistry data raised the possibility of a methodological interference. A thorough evaluation of the main causes of interference was conducted in the laboratory to exclude the presence of interference in TSH and FT4. Finally, different interfering agents were identified, which affected free thyroid hormone and TSH determination.

Abstract

Resumen Objetivos Los inmunoensayos utilizados para evaluar la función tiroidea son vulnerables a diferentes tipos de interferencias que pueden afectar a las decisiones clínicas. Caso clínico Presentamos el caso de una mujer de 37 años que acudió a nuestro hospital para su revisión médica anual por presentar hipotiroidismo iatrogénico tras tratamiento con radioiodo. La paciente se encontraba asintomática, sin signos sugestivos de enfermedad tiroidea. Sin embargo, en el estudio analítico destacó el resultado de su perfil tiroideo: TSH 7,75 mU/L, FT4 > 7,7 ng/dL. Conclusiones Ante resultados discordantes entre la clínica y el patrón bioquímico obtenido, se consideró la posibilidad de una interferencia metodológica. Se realizó una revisión detallada de las principales causas de interferencias y se realizaron una serie de procedimientos por parte del laboratorio para descartar la presencia de interferencias en la TSH y la FT4, comprobándose finalmente que había distintos agentes interferentes que estaban afectando a las mediciones de las hormonas tiroideas libres y a la TSH de la paciente.

Abstract

Objectives The objective of this study was to highlight the role of the clinical laboratory and the relevance of reporting the case immediately to the unit of hematology for the diagnosis and early administration of treatment in the presence of such an urgent hematologic disease as thrombotic thrombocytopenic purpura (TTP). Case presentation An elderly patient was referred to the emergency department of our hospital by his general practitioner for speech difficulty, facial asymmetry and weakness in the upper limb. Stroke code was activated. However, laboratory findings (anemia, thrombocytopenia, elevated creatinine, total bilirubin and LDH, negative direct Coombs test) and presence of schistocytes in the peripheral blood smear test were consistent with a completely different diagnosis: TTP thrombotic microangiopathy. Conclusions The first diagnostic approach of left hemispheric stroke was not confirmed in the laboratory, with findings of nonautoimmune hemolytic anemia, thrombocytopenia without apparent cause and presence of schistocytes. We should not forget that the clinical manifestations of this condition are widely variable and may include multiorganic dysfunction. Although confirmation of diagnosis is based on ADAMTS-13, its associated high mortalitiy requires immediate treatment on mere suspicion.

Caso clínico Varón de edad avanzada derivado a Urgencias hospitalarias por su médico de Atención Primaria por trastorno de emisión del lenguaje, asimetría facial y disminución de fuerza en extremidad superior, por lo que se activa el código ictus. Sin embargo, los hallazgos de laboratorio (anemia y trombocitopenia, creatinina, bilirrubina total y LDH elevadas, test de Coombs directo negativo) y presencia de esquistocitos en el frotis de sangre periférica, acaban derivando en un diagnóstico completamente diferente: microangiopatía trombótica tipo PTT. Conclusiones La primera orientación diagnóstica de ictus hemisférico izquierdo fue rechazada ante los signos de anemia hemolítica no autoinmune, trombocitopenia sin causa aparente y presencia de esquistocitos. No debemos olvidar que esta patología puede cursar con afectación multiorgánica y características clínicas muy variables. Aunque su diagnóstico definitivo se basa en la determinación de actividad de ADAMTS-13, debido a la elevada mortalidad es necesario instaurar tratamiento de forma inmediata tras su sospecha.

Objective: Advances in Laboratory Medicine / Avances en Medicina de Laboratorio (Adv Lab Med) is the official journal of the Spanish Society of Laboratory Medicine (SEQC^ML). Adv Lab Med publishes contributions in the field of medical sciences, including Biochemistry, Hematology, Toxicology, Microbiology and Parasitology, Immunology, and Genetics. Articles focusing on the choice, performance or interpretation of clinical tests are especially welcome.
Adv Lab Med publishes quarterly. All contributions submitted for publication in Adv Lab Med are single-blind peer reviewed by at least two experts in the field, selected and invited by the Editor-in-Chief.
